Mobile Notary:

First Call has a notary on staff and does not charge to have documents notarized. There are also other community resources available to have documents notarized. Examples of documents that can be notarized are advance health care directives, medical documents, power of attorney documents, etc.

 

Caregiver Resources:

  • Lifestyles allows families to look for senior living homes for their loved ones and to hire caregivers in their home.

    • Senior living homes include independent living, assisted living, care homes, memory care, and nursing homes

    • Caregivers can provide personal care services, homemaker services, companionship. This can include basic personal hygiene assistance, meal preparation, light housekeeping, medication reminders, and transportation.

Bereavement Groups:

First Call Hospice has a bereavement counselor on staff to assist with grief and loss. Contact office to get in contact with bereavement counselor, appointment only.

Children Bereavement Group:

Sutter Health – Children’s Bereavement Art Group:

Offers an art support group for children between 4 – 17 years old who are grieving from a loss of a loved one. The use of art and other creative activities aid children to express grief in a healthy self-expression.

Adult Bereavement Groups:

Sutter Health Adult Grief-Related Services:

Sutter Health offers bereavement support groups for adults related to grief, spouse/partner loss, parent loss, and child loss. Services are located in Sacramento and in Roseville.

 Widowed Persons Association of California in Sacramento:

Nonprofit organization dedicated to helping widowed individuals overcome grief learn to live and laugh again.

Advance Health Care Directive Form:

This form allows you to select an individual to make healthcare decisions for you when you are no longer capable or allows the individual to make healthcare decisions for you even though you are still capable. This form also allows you to give specific instructions about any aspect of your healthcare. The links below have more information on advance directives and you can choose any form to fill out.

 

Family and Medical Leave Act (FMLA):

FMLA forms allow employee leave due to serious health conditions or leave due to caregiving for a family member with a serious health conditions
